Only fill this form out if you plan on paying at pick up with cash or a check.

Invoices will be sent within 24 hours after receiving an order.

If changes need to be made to your order before April 1, 2025, Do not place another order. Call Ole Star Graphix (806)318-1128.

No changes can be made after 12:00PM on April 1,2025.

You are responsible for payment for each item placed on any form you submit after April 1, 2025 at 12:00pm.

Orders will need to be picked up April 7-9, 2025. Ole Star Graphix will be closed April 10-13, 2025. If you fail to pick up your order you are still responsible for payment.
